Insurance is not accepted at this time.

 

A superbill can be provided upon request, which you may submit to your insurance carrier for possible reimbursement, depending on your plan and benefits.

 

Questions regarding coverage, reimbursement, or eligibility should be directed to your insurance provider directly.

This office does not contact insurance companies on a patient’s behalf.

Package options are discussed after your initial visit, once we’ve had a chance to assess what’s appropriate for your body, goals, and timeline.
